Preparing For Your Appointment

STEP ONE
- Sign and email your CONSENT and RATES & POLICY forms to Rebecca.
STEP TWO
- Fill out your horse’s HEALTH HISTORY QUESTIONNAIRE.
STEP THREE
- Before each appointment provide at least one but not more than 3 chief complaints or areas of concentration where you are requesting help. Provide this information to Rebecca at the beginning of your appointment. This is super important for the success of your treatment.
- Checkout the available services and make a request if there is a service you are particularly interested in.
STEP FOUR
- Before each appointment rate your horse’s mental, physical, or emotional health condition on a scale of 1 -10 (one being very mild to 10 being very strong). You can tell Rebecca this verbally and she will mark it in your horse’s chart.
